At Chain Break, we provide recovery-based, flexible, and community-focused NDIS support for individuals living with severe or persistent mental health challenges.

Our goal is to help you build independence, develop skills, and regain control over your life. We pride ourselves on our non—judgmental approach by providing supports with real life experience that get it and get you.

Using a coaching model, our support workers combine professional expertise with lived experience, offering compassionate, practical support during regular visits in your home, local community or anywhere else suitable for you. We work alongside you to enhance self-advocacy, increase confidence, and develop strategies that support long-term recovery.

The level of support varies based on individual needs and NDIS plan resouces. This may range from weekly check-ins to daily, intensive support, ensuring you receive the right level of care for your situation.

Our support can assist with:
